Assurant - Plan Information
Who should consider Assurant? Healthy U.S. residents between the
ages of eleven months and 64 years of age or healthy foreign residents
who have been living in the U.S. for at least one year at the time
of enrollment with proof of an Alien Registration Receipt Card, visa,
or other appropriate documentation. With this plan you can choose
flexible deductibles of $250.00, $500.00, $1,000.00, $2,500.00, or
more. This plan also offers you the freedom to choose your own doctors
and hospitals, provides prescription drug coverage, and both in-hospital
and out-patient benefits. You will only need to answer a few short
questions to apply.

terms, benefits, and exclusions can be found in the insurance contract. Assurant
temporary plans are currently not available in the
following states: HI, MA, NJ, NY, and VT.

Celtic Insurance Company - Plan Information
Who should consider Celtic? Healthy U.S. residents between the ages
of six months and 64 ½ years of age or healthy foreign residents
who have been living in the U.S. for at least three years with proof
of an Alien Registration Receipt Card or “Green Card.” With
this plan, you can choose one of three deductibles at $500.00, $1,000.00
or $2,500.00. This plan also offers major medical insurance, high-quality
benefits including in-patient and out-patient coverage, doctor office
visits, surgical fees, and laboratory tests. You will only need to
answer a few short questions to apply.

temporary plans are currently not available in the
following states: AK, CO, HI, KY, MA, MI, MN, ND, NH, NJ, NY, OR,
